Change of Name

All requested changes of student information must be submitted in writing and accompanied by proper documentation.

To change your name on your academic records with IIT, please submit legal documentation (i.e. driver's license, court order ) supporting the change with a signed request form and return the items to the Office of the Registrar:

Change of Address

To change your local, permanent, work, billing, or next-of-kin addresses please go to the IIT Web for Students site, login to Student Services and go to "Update Addresses".

Change of Student Identification Number/Social Security Number

To change a Student Identification Number please submit a written and signed request and return it with a copy of your social security card to:
